Lt. David Formeza, 45, of Perth Amboy, passed away suddenly at his residence on October 28, 2021. Born in Perth Amboy, he was a lifelong resident. Lt. Formeza had served the Perth Amboy Police Department for 18 years and was highly regarded and loved by the Perth Amboy Department Community. Lt. Formeza was assigned to the nightshift A Squad Patrol Bureau and a former member of the Perth Amboy Police SWAT team.

Lt. Formeza was predeceased by his parents, Teudis Noel and Ubaldina Formeza.

Lt. Formeza is survived by his brother, Ted A. Formeza, and his cousins, Odalys, Maria Elena, Carlos, Marina and Maria.

Visitation will be held on Tuesday, November 9, 2021, at Flynn and Son Funeral Home, 424 East Ave., Perth Amboy from 2-4 PM and 7-9 PM.

Funeral services will begin at 9:15 AM on Wednesday, November 10, 2021, From Flynn and Son Funeral Home, 424 East Ave., Perth Amboy followed by a 10:00 AM Funeral Liturgy at St. Stephen's Church of John Paul II Parish, Perth Amboy. Interment will follow in Alpine Cemetery.

It is the families request that everyone attending the viewing and the church Mass to please wear a mask while indoors.
